HealthCompare In-Store Agent Lead Form Instructions

This form is for agents only - do not hand it to the consumer. Use your own email address (the same one linked to your HubSpot account) when submitting the lead.
 
Always ask the consumer for permission to be contacted and record their TCPA consent using the checkbox provided.
 
After submitting the lead, you’ll be directed to a landing page to choose the product type (“Under 65” or “Medicare”). For Allstate products, click Login on the site and enter your NPN to complete enrollment, or process the application through ENG as usual. For Medicare products, use the carrier platform (or another contracted platform) and only present plans you’re authorized to sell. After completing a sale, return to HubSpot and add the policy details to the lead record.
